厌氧瓶的使用对血液微生物培养的重要意义

2019年第2期Vol.29No.2检验检疫学刊JOURNALOFINSPECTIONANDQUARANTINE1前言血流感染是医院常见的感染性疾病袁菌血症的病死率可达20%耀50%[1袁2]袁尽管随着病原学诊断技术飞速发展袁免疫学尧分子生物学方法得到广泛应用袁但是血培养仍然是菌血症临床诊断诊的标准袁血培养也是临床微生物实验室最重要的工作内容之一袁临床往往通过血培养结果来判断病患是否全身感染袁如菌血症尧败血症尧感染性心内膜炎等遥肺部感染是临床感染常见病种袁但是临床医生往往以咳痰尧抽吸痰尧支气管灌洗液和支气管肺泡灌洗液作为寻找病原体的首选标本[3]袁然而袁血培养才是诊断细菌性肺炎的首选标本遥血培养结果的影响因素中最重要的就是检验前的血液采集过程袁而该过程需要临床医生根据经验有效地控制采集时机袁护理人员经过培训采取正确的采集方式并控制血量遥其中袁厌氧瓶的使用具有十分重要的意义遥常熟市医学检验所临厌氧瓶的使用对血液微生物培养的重要意义邓一脉马月琴戈惠丽侯一立何敏霞蒋廷旺渊常熟市医学检验所江苏常熟215500冤摘要为了分析常熟地区送检的7753瓶血培养的结果了解厌氧瓶的使用对于病原菌检出的影响袁收集2018年1月要8月的7753瓶血培养的结果遥分析厌氧瓶使用前后袁临床单日送检血培养套数尧病原菌种类和占比的变化曰同一病人送检的血培养阳性结果袁统计分析需氧瓶和厌氧瓶报阳时间的差异遥结果显示袁7753瓶血培养标本中单瓶血培养占比51.42%袁病原菌检出率5.35%袁单套血培养占比33.85%袁病原菌检出率9.56%袁双侧双瓶血培养占比14.10%袁病原菌检出率15.41%遥对于同一病人同时送检的血培养袁59%厌氧瓶比需氧瓶预先报阳袁其中89%以上厌氧瓶在6h以内比需氧瓶预先报阳遥因此袁单侧单瓶血培养袁对病原菌捐出率有显著影响遥关键词血培养曰厌氧瓶曰细菌中图分类号R446.5ClinicalSignificanceoftheUseofAnaerobicBottleinBloodCultureDENGYimai,MAYueqin,GEHuili,HOUYili,HEMinxia,JIANGTingwang(InstitutionofLaboratoryMedicine,Changshu袁Jiangsu,215500,China)Abstract院Inordertoanalyzetheresultsof7753bottlesofbloodculturesenttoChangshuareatounderstandtheinfluenceoftheuseofanaerobicbottlesonpathogenicbacteriadetection袁theresultsof7753bottlesofbloodculturefromJanuarytoAugust2018werecollected.Toanalyzethechangesofthenumberofbloodculturesets,pathogenicbacteriaspeciesandproportionbeforeandaftertheuseofanaerobicbottles;toanalyzethepositiveresultsofbloodculturefromthesamepatient,andtoanalyzethedifferenceofthetimeofreportingthesunbetweenaerobicbottlesandanaerobicbottles.Theresultsshowthatamong7753bloodculturesamples,51.42%aresinglebottlebloodculture,5.35%arepathogenicbacteria,33.85%aresinglebloodculture,9.56%arepathogenicbacteria,14.10%arebilateralbloodcultureand15.41%arepathogenicbacteria.Forthesamepatient'sbloodculture,59%oftheanaerobicbottlesreportedpositiveresultsinadvancethantheaerobicbottles,andmorethan89%oftheanaerobicbottlesreportedpositiveresultsinadvancewithin6hoursthantheaerobicbottles.So,unilateralsinglebottlebloodculturehasasignificanteffectontherateofpathogenicba
